Job summary

MaineGuide is seeking a Licensed Social Worker to support residents facing complex challenges in a long-term care setting. You will assess needs, develop care plans, and coordinate services to promote well-being and independence.

In this in-person role, you will utilize motivational interviewing and crisis intervention skills to guide residents toward meaningful goals while collaborating with families and care teams. Expected hours are 20 per week.

Qualifications

• Must be licensed social worker in good standing.
• Experience in long-term care and behavioral health.
• Strong communication and relationship-building skills.
• Experience with crisis intervention is preferred.

Responsibilities

• Conduct comprehensive assessments of residents' social, emotional, and behavioral health needs to inform personalized care strategies
• Schedule and attend weekly care plan meetings
• Utilize motivational interviewing techniques to motivate residents toward positive change and goal achievement
• Proven experience in social work and long term care, with a focus on behavioral health
• Demonstrated success in crisis intervention and crisis management within community or clinical settings
• Prior experience in addiction counseling or substance abuse treatment programs preferredStrong communication skills with the ability to build trust and rapport with diverse populations
