Responsibilities

• Provide social services addressing residents' emotional, social, and psychosocial needs.
• Advocate for residents and promote dignity, choice, independence, and quality of life.
• Participate in care planning with the interdisciplinary team to develop and support individualized resident goals.
• Communicate with residents, families, staff, and other involved professionals regarding resident needs and concerns.
• Assist with discharge planning, referrals, resources, and coordination of services as appropriate.
• Maintain accurate and timely documentation in accordance with facility policies and applicable regulations.

The Social Worker plays an important role in advocating for residents, coordinating services, supporting care planning, and helping residents navigate their stay. As a Social Worker, you will work closely with residents, families, nursing staff, and the interdisciplinary team to promote quality of life and positive resident outcomes.

Key Responsibilities
