Job summary

Terence Cardinal Cooke Health in New York seeks a Day Shift Full-Time Social Worker with an MSW to plan, organize, and implement social work services for residents and families. You will coordinate discharge planning and home care, and maintain ties with referring agencies and community resources.

Under supervision and in alignment with in-service training, you will participate in resident care services and discharge planning to support elders and their families in our Continuing Care Community.

Qualifications

• Master of Social Work (MSW) required.
