Job summary

ArchCare, part of the Archdiocese of New York, is seeking a Social Worker to support residents in a continuing care setting in Bronx, NY. Responsibilities include psychosocial assessments, discharge planning, and coordinating services to promote emotional and physical well-being.

Require a Bachelor's in social work, one year of related experience; MSW preferred. Bilingual English/Spanish is a plus. Full-Time, day shift, 9am–5pm, with competitive benefits and paid time off.

  • Complete documentation in accordance with departmental requirements.

Job Description
Essential Functions
  • Respond to resident call bells and refer resident and family requests to appropriate healthcare team members.
  • Interview residents and family members to obtain information for comprehensive psychosocial assessments.
  • Coordinate and monitor available services for individual residents to support an optimum level of emotional, physical, and psychological well-being and independence.
  • Identify and resolve problems regarding room changes, transfers, and discharge with appropriate departments and administration.
  • Facilitate communication with other departments on behalf of residents, including medical, nursing, dietary, rehabilitation, and psychiatric services.
  • Meet with residents and families and initiate discharge-planning activities.
  • Participate in Comprehensive Care Plan Meetings and team conferences and formulate comprehensive care plans.
  • Complete documentation in accordance with departmental requirements.
  • Document referrals received for resident intervention in a timely manner.
  • Consistently follow nursing home and departmental policies in the performance of duties.
  • Provide supportive counseling and mental health support to residents only. Provide family members with appropriate information, support, education, and referrals; mental health counseling is not provided to family members.
  • Participate, when requested by residents, in the ongoing functioning of the resident and family council.
  • Initiate and facilitate meetings among residents, families, and staff to promote a fuller understanding of institutionalized residents and joint participation in improving residents' emotional and physical well-being.
  • Coordinate and facilitate referrals for residents to outside resources for services not available at the facility.
  • Assist in the care and maintenance of department equipment and supplies.
  • Attend meetings as required and participate in committees as directed.
  • Enhance professional growth and development through educational programs, current literature, in-service meetings, workshops, and similar activities.
Education & Experience

Formal Education: Bachelor's degree in social work (BSW) or another related health field.

Experience: One year of related experience.

Minimum Knowledge: Advanced knowledge of a specialized or technical field, or thorough knowledge of the practices and techniques of a professional field.

Language Ability: Ability to read and interpret safety rules, operating and maintenance instructions, and procedure manuals; write routine reports and correspondence; and speak effectively before groups of customers or employees.

Licensure: No professional social work license is required for this staff Social Worker position.

Education Desired: Master's degree in social work (MSW) preferred.

Language Desired: Bilingual English/Spanish preferred but not required.
