Job summary

German Centre Home in Boston, MA is seeking a Social Worker to perform psychosocial assessments, counseling, and discharge planning within an interdisciplinary team.

Required: MSW/BSW and MA LSW/LCSW/LICSW license; English fluency; experience in long-term care preferred. Salary is $82,000–$92,000 annually; schedule is Monday–Friday, no weekends or holidays.

Qualifications

• Bachelor or Master of Social Work (BSW or MSW) is required.
• Current, unencumbered MA LSW, LCSW, or LICSW license.
• Prior experience in a long-term care setting strongly preferred.
• Ability to communicate fluently in English – both verbally and written.

• Provide counseling, intervention, and ongoing support to residents and their families while advocating for resident rights and quality of life.
• Collaborate with an interdisciplinary care team to coordinate services, facilitate care conferences, and ensure resident-centered care.
• Connect residents and families with community resources and post-discharge support services to promote successful care transitions.

Qualifications for Success:

• Bachelor or Master of Social Work (BSW or MSW)
• Current, unencumbered MA LSW, LCSW, or LICSW license
• Prior experience in a long-term care setting strongly preferred
