Job summary

German Centre Home is seeking a Social Worker in Massachusetts to provide psychosocial assessments, counseling, and discharge planning for residents. The role supports an interdisciplinary team to ensure resident-centered care and transitions.

Qualifications

• Bachelor or Master of Social Work (BSW or MSW)
• Current, unencumbered MA LSW, LCSW, or LICSW license
• Prior experience in a long-term care setting strongly preferred
• Ability to communicate fluently in English – both verbally and written
