Job summary

JGS – Leavitt Family Jewish Home in Massachusetts is seeking a part-time Social Worker (24 hours/week) with pay between $30.00 and $34.00 per hour, based on experience.

Responsibilities include psychosocial assessments, counseling, and coordination with an interdisciplinary team to support resident rights, care planning, and smooth post-discharge transitions.

Qualifications

• Bachelor or Master of Social Work (BSW or MSW) required.
• Current MA LSW, LCSW, or LICSW license required.
• Long-term care experience preferred.
• English communication skills required.

Delivering Solutions that Matter:

• Conduct comprehensive psychosocial assessments and develop individualized care plans to address residents' emotional, social, and discharge planning needs.
• Provide counseling, intervention, and ongoing support to residents and their families while advocating for resident rights and quality of life.
• Collaborate with an interdisciplinary care team to coordinate services, facilitate care conferences, and ensure resident-centered care.
• Connect residents and families with community resources and post-discharge support services to promote successful care transitions.

Qualifications for Success:

• Bachelor or Master of Social Work (BSW or MSW)
• Current, unencumbered MA LSW, LCSW, or LICSW license
• Prior experience in a long-term care setting strongly preferred
• Ability to communicate fluently in English – both verbally and written
