Job summary

Chelsea Jewish Lifecare in Peabody, MA is seeking a Social Worker (PRN) at the Jeffrey & Susan Brudnick Center for Living. The role includes psychosocial assessments, counseling, and discharge planning to support residents' emotional and social well-being.

Licensed MA LSW/LCSW/LICSW required; MSW/BSW preferred. Long-term care experience is preferred. This PRN position offers competitive pay from $30 to $63 per hour, based on experience.

Qualifications

• Bachelor or Master of Social Work (BSW or MSW).
• Current, unencumbered MA LSW, LCSW, or LICSW license.
• Prior experience in long-term care preferred.
• Fluency in English (spoken and written).

Responsibilities

• Conduct psychosocial assessments and develop individualized care plans addressing residents' emotional and discharge planning needs.
• Provide counseling, intervention, and ongoing support to residents and families while advocating for rights and quality of life.
• Collaborate with an interdisciplinary care team to coordinate services, facilitate care conferences, and ensure resident-centered care.
• Connect residents and families with community resources and post-discharge support services for successful transitions.
