Job summary

JGS – Leavitt Family Jewish Home in Massachusetts is seeking a dedicated Social Worker to support residents and families, with 24 hours per week at a part-time schedule and an hourly rate of $30 to $34, based on experience.

You will conduct psychosocial assessments, develop care plans, provide counseling, and collaborate with an interdisciplinary team to ensure resident-centered care and smooth transitions.

Qualifications

• Bachelor or Master of Social Work (BSW or MSW).
• Current, unencumbered MA LSW, LCSW, or LICSW license.
• Prior experience in a long-term care setting preferred.
• Ability to communicate fluently in English (verbal and written).

Responsibilities

• Conduct psychosocial assessments and develop individualized care plans.
• Provide counseling and support to residents and families; advocate for rights.
• Collaborate with care team to coordinate services and care conferences.
• Connect residents with community resources and post-discharge support.

Delivering Solutions that Matter:

• Conduct comprehensive psychosocial assessments and develop individualized care plans to address residents' emotional, social, and discharge planning needs.
• Provide counseling, intervention, and ongoing support to residents and their families while advocating for resident rights and quality of life.
• Collaborate with an interdisciplinary care team to coordinate services, facilitate care conferences, and ensure resident-centered care.
• Connect residents and families with community resources and post-discharge support services to promote successful care transitions.

Qualifications for Success:

• Bachelor or Master of Social Work (BSW or MSW)
• Current, unencumbered MA LSW, LCSW, or LICSW license
• Prior experience in a long-term care setting strongly preferred
