The Jeffrey & Susan Brudnick Center for Living in Peabody, MA is seeking a dedicated Social Worker (PRN) to provide psychosocial assessments, counseling, and care coordination for residents and families. This role collaborates with an interdisciplinary team to support resident-centered planning and smooth transitions.

Qualified candidates hold a BSW or MSW and MA LSW/LCSW/LICSW license, with experience in long-term care and strong English communication.

Qualificações

• Bachelor or Master of Social Work (BSW or MSW) required.
• Current, MA LSW/LCSW/LICSW license in good standing.
• Prior long-term care experience preferred.
• Fluent in English (spoken and written).

• Conduct comprehensive psychosocial assessments and develop individualized care plans to address residents' emotional, social, and discharge planning needs.
• Provide counseling, intervention, and ongoing support to residents and their families while advocating for resident rights and quality of life.
• Collaborate with an interdisciplinary care team to coordinate services, facilitate care conferences, and ensure resident-centered care.
• Connect residents and families with community resources and post-discharge support services to promote successful care transitions.

Qualifications for Success:

• Bachelor or Master of Social Work (BSW or MSW)
• Current, unencumbered MA LSW, LCSW, or LICSW license
• Prior experience in a long-term care setting strongly preferred
• Ability to communicate fluently in English - both verbally and written
