Job summary

German Centre is seeking a Social Worker in Boston to provide psychosocial assessments, counseling, and discharge planning for residents in a Skilled Nursing Facility. This full-time role offers a Monday–Friday schedule with no weekends or holidays.

Qualifications

• BSW or MSW degree required.
• Current MA LSW, LCSW, or LICSW license in MA.
• Experience in a Skilled Nursing Facility preferred.
• Excellent English communication, both written and spoken.

Responsibilities

• Provide counseling and ongoing support to residents and families, advocating for quality of life.

$82,000 - $92,000/annually (based off experience)

Delivering Solutions That Matter

• Conduct comprehensive psychosocial assessments and develop individualized care plans to address residents' emotional, social, and discharge planning needs.


• Provide counseling, intervention, and ongoing support to residents and their families while advocating for resident rights and quality of life.


• Collaborate with an interdisciplinary care team to coordinate services, facilitate care conferences, and ensure resident-centered care.


• Connect residents and families with community resources and post-discharge support services to promote successful care transitions.

Qualifications For Success

• Bachelor or Master of Social Work (BSW or MSW)


• Current, unencumbered MA LSW, LCSW, or LICSW license


• Prior experience working in a Skilled Nursing Facility is required.


• Ability to communicate fluently in English – both verbally and written
