Job summary

Valley-Hill-Health-and-Rehab is seeking a Social Services professional to assist the Director in meeting residents' emotional and social needs in a skilled nursing setting. You will plan, organize, and oversee the Community’s Social Services Department, ensuring compliance with federal, state, and local standards.

Requirements include a high school diploma or equivalent, a current unencumbered social work license if required, and experience in long-term care and behavioral health.

Qualifications

• Must possess a high school diploma or equivalent.
• Possession of a current, unencumbered license to practice as a Social Work within the state in which you work, if required by state/federal regulations.
• Must have experience in Long Term Care as well as behavioral health
• Experience working in healthcare, either social services, direct care or hospitality, is preferred.
• Must be able to communicate in English and have the ability to follow verbal and written instructions.

Responsibilities

• Assist the Director to ensure that the medically related emotional and social needs of the resident are met on an individual basis.
• Help plan, organize, implement, evaluate and direct the Social Services Department in accordance with applicable standards and policies.
• Follow health, sanitary and infection control policies and maintain established standards of practice.

Position Summary:

The primary purpose of your job is to assist the Director to ensure that the medically related emotional and social needs of the resident are met/maintained on an individual basis. You willassist in planning, organizing, implementing, evaluating, and directing the overall operation of the community’s Social Services Department inaccordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures.Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the community's administration and Social Services Policies and Procedures.

Education and Qualifications:

• Must possess a high school diploma or equivalent.

• Possession of a current, unencumbered license to practice as a Social Work within the state in which you work, if required by state/federal regulations.

• Must have experience in Long Term Care as well as behavioral health
