Job summary

Concordia at Villa St. Joseph Skilled Nursing is seeking a compassionate and motivated Social Worker to join our care team.

Qualifications

• Bachelor’s Degree in Social Work or related field.
• At least two years of supervised social work experience in a health care setting.
• Strong documentation and communication skills with residents, families, staff, and partners.

Responsibilities

• Admissions: support new residents and families during admissions.
• Care planning: participate in interdisciplinary care conferences and update resident care plans.
• Discharge planning: coordinate follow-up services.
• Documentation: complete notes and reports in compliance with regulations.

Qualifications of aSocial Worker:

• Bachelor’s Degree in Social Work or Human Relations field (sociology, counseling, psychology, special education, rehabilitation, etc.)
• At least two years of supervised social work experience in a health care setting required
• Strong documentation skills, communication skills with residents, families, staff, and community partners and ability to work independently and as part of a team

Key Responsibilities of aSocial Worker:

• Admissions:Support new residents and families during the admission process; complete social history, care plans, and discharge planning documentation.
• Care Planning:Participate in interdisciplinary care conferences and update resident care plans and involve families in care planning, offer support, and provide guidance on community services.
• Discharge Planning:Coordinate follow-up services and provide resources to residents and families.
• Documentation:Complete timely and accurate notes, assessments, and reports in compliance with regulations.
