Job summary

Brookside Commons in Farmington Hills, MI is seeking a Social Worker in long-term care to advocate for residents, coordinate admissions, care planning, and transitions, and support families with empathy.

You will participate in interdisciplinary conferences, ensure compliant documentation, and help residents navigate discharge planning while maintaining resident-centered care.

What You'll Do

Qualifications

• Previous experience in a skilled nursing facility or long-term care setting is strongly preferred.
