Qualifications

• Bachelor's degree required; Master's degree strongly preferred.
• LSW licensure required; LCSW or LICSW strongly preferred.
• Minimum of two years' experience in a skilled nursing facility.
• Experience with short-term rehabilitation and discharge planning.
• Excellent communication, documentation, and assessment skills.
• Strong interpersonal and organizational abilities.
• Genuine concern for and interest in working with people.
• Ability to work with residents, families, healthcare professionals, and community service providers.

Responsibilities

• Build positive relationships between residents and staff to support each resident's needs.
• Identify options and resources that meet physical, emotional, and psychosocial needs.
• Maintain communication with families regarding health changes, goals, and care planning.
• Make referrals and coordinate services with outside agencies and community resources.

• Participate on the interdisciplinary team to support comprehensive resident care.

Responsibilities
