Job summary

Riverside-Commons in Michigan is seeking a dedicated Social Worker to advocate for residents and guide families through admissions, care planning, and transitions within a busy long-term care community.

You will collaborate with nursing, therapy, physicians, and other departments to ensure quality, resident-centered care and compliant documentation, while supporting families during difficult transitions.

Qualifications

• Licensed Social Worker (LMSW or LLMSW) preferred, but experienced non-licensed candidates with long-term care social services experience are encouraged to apply.
• Previous experience in a skilled nursing facility or long-term care setting is strongly preferred.
• Excellent communication, organization, and problem-solving skills.
• Ability to prioritize and thrive in a fast-paced environment.
• Compassionate, positive attitude with a strong commitment to resident-centered care.

Responsibilities

• Advocate for residents' emotional, psychosocial, and discharge planning needs.
