Concordia at Villa St. Joseph Skilled Nursing is seeking a compassionate and motivated Social Worker to join our care team.

The Social Worker provides support to residents and their families in accordance with Federal and State regulations, as well as facility policies, to ensure psychosocial needs are met

Qualifications

• Bachelor’s Degree in Social Work or related field.
• At least two years of supervised social work experience in a health care setting.
• Strong documentation and communication skills with residents, families, staff, and partners.

Responsibilities

• Admissions: support new residents and families during admissions.
• Care planning: participate in interdisciplinary care conferences and update resident care plans.
• Discharge planning: coordinate follow-up services.
• Documentation: complete notes and reports in compliance with regulations.
