Job summary

Concordia Lutheran Ministries' Concordia at Villa St. Joseph is seeking a compassionate and motivated Social Worker to join our care team.

This role supports residents and families, ensuring psychosocial needs are met with dignity and respect, in accordance with federal and state regulations. You will participate in admissions, care planning, and discharge planning, document thoroughly, and collaborate with an interdisciplinary team.

Qualifications

• Bachelor’s Degree in Social Work or Human Relations field (sociology, counseling, psychology, special education, rehabilitation, etc.)
• At least two years of supervised social work experience in a health care setting
• Strong documentation skills, communication skills with residents, families, staff, and community partners and ability to work independently and as part of a team

Responsibilities

• Admissions: Support new residents and families during the admission process; complete social history, care plans, and discharge planning documentation.
• Care Planning: Participate in interdisciplinary care conferences and update resident care plans and involve families in care planning, offer support, and provide guidance on community services.
• Discharge Planning: Coordinate follow-up services and provide resources to residents and families.
• Documentation: Complete timely and accurate notes, assessments, and reports in compliance with regulations.
