Qualifications

• Degree or certification in Social Work or related field as required by state and facility regulations
• Previous experience in a skilled nursing, long-term care, senior living, or healthcare setting preferred
• Strong communication, organization, and interpersonal skills
• Compassionate and resident-centered approach
• Ability to work effectively with residents, families, and an interdisciplinary care team
• Strong attention to detail and ability to maintain confidentiality

Responsibilities

• Provide social services and support to residents as a Social Worker, addressing individual emotional, social, and psychosocial needs
• Advocate for residents and help promote dignity, choice, independence, and quality of life
• Participate in care planning and collaborate with the interdisciplinary team to develop and support individualized resident goals
• Communicate with residents, families, staff, and other involved professionals regarding resident needs and concerns
• Assist with discharge planning, referrals, resources, and coordination of services as appropriate
• Maintain accurate and timely documentation in accordance with facility policies and applicable regulations
