Job summary

PACE of Southwest Michigan seeks a Social Work Supervisor to lead the Social Work team across the St. Joseph and Niles PACE campuses. You will supervise licensed and non-licensed staff, guide case management, and ensure documentation aligns with CMS requirements.

You will support interdisciplinary collaboration, mentor staff, and help strengthen psychosocial care for participants in a high-quality, team-based care model.

Qualifications

• Master’s degree in Social Work (MSW) from CSWE-accredited program.
• Active LMSW license in the State of Michigan.
• Minimum of 1 year of experience working with the frail elderly or long-term care population.

Responsibilities

• Provide supervision and mentorship to Social Work team across St. Joseph and Niles campuses.
• Coach and develop licensed and non-licensed staff through regular feedback and professional development.
• Monitor continuing education, licensing requirements, and employee documentation.
• Coordinate and strengthen social work processes, workflows, and case management practices.
• Ensure social work services are integrated into the Interdisciplinary Team (IDT) and aligned with the PACE model of care.
• Serve as a resource and advocate for participant-centered care, including advance care planning.
• Maintain proficiency in social work case management and guidance on complex clinical situations.
• Manage a limited participant caseload as needed.
• Support and train team members in the use of the Electronic Health Record (EHR).
• Develop, implement, and reinforce policies and documentation standards per CMS and SAA.
