Brio Living Services in St. Joseph, MI, seeks a Social Work Supervisor to lead the Social Work team across the St. Joseph and Niles PACE campuses. You will supervise licensed and non-licensed staff, strengthen case management practices, and ensure seamless integration with the interdisciplinary team.

The role focuses on professional development, compliance with PACE requirements, and advancing whole-person care for participants. Strong leadership and collaborative skills are essential.

Competenze

• Master’s degree in Social Work (MSW)
• Active LMSW license in the State of Michigan
• Minimum of 1 year of experience with frail elderly or long-term care population

Mansioni

• Provide leadership, supervision, and ongoing support to Social Work team across St. Joseph and Niles campuses.
• Coach and develop licensed and non-licensed staff with regular feedback and professional development.
• Monitor continuing education, licensing requirements, and employee documentation.
• Strengthen social work processes, workflows, and case management.
• Ensure services align with the PACE model and CMS/SAA requirements.
• Collaborate with interdisciplinary team to deliver holistic participant care.
• Support and train staff in EHR usage and documentation standards.
• Participate in quality improvement and organizational initiatives.
