PACE Southeast Michigan is seeking a Social Worker Lead (LMSW) to join our Sterling Heights site onsite. You will assess needs, coordinate social services, and develop care plans as part of the IDT, while providing daily supervision to junior social workers.

The role emphasizes bio-psychosocial assessments, counseling using CBT/Motivational Interviewing techniques, and ensuring timely EHR documentation while maintaining compliance with regulations.

Qualifications

• Master’s degree in social work from an accredited program.
• Licensed to practice in Michigan; LMSW required.
• Minimum three years clinical social work experience, preferably in geriatrics.
• Experience in supervision/leadership preferred; indicates leadership potential.

Responsibilities

• Lead daily clinical management and supervision of social workers.
• Assess participant needs and coordinate social services.
• Conduct bio-psychosocial assessments and care planning.
• Collaborate with Interdisciplinary Team to determine eligibility and care plans.
• Ensure documentation in EHR is accurate and timely.
