PACE Southeast Michigan in Southfield is seeking a Social Work Lead (LMSW) to assess, coordinate, and deliver social services for participants and families. The LMSW will participate in the IDT and lead daily clinical management and supervision of social workers at the center.

The role includes developing treatment plans, conducting assessments, advocating for care, ensuring documentation in the EHR, and guiding care planning while maintaining regulatory compliance and quality improvement.

Competenze

• Master’s Social Worker from an accredited program and LMSW license in Michigan.
• Minimum three years as a clinical social worker, preferably geriatrics.
• Experience in supervision/leadership and coordinating care with an IDT.

• Provide daily clinical management, supervision, and oversight of social workers.
• Assess, coordinate, and deliver social services for participants and families.
• Lead interdisciplinary team participation and care planning.
• Develop treatment plans and conduct bio-psychosocial assessments at enrollment and follow-ups.
