Job summary

Allegheny Health Network is seeking an experienced social work professional to advance care coordination and patient advocacy within the Care Management team. You will collaborate with clinicians, health plans, and community partners to improve care quality and patient experience across the continuum.

The role emphasizes social determinants of health, safe discharge planning, and crisis intervention, including end-of-life discussions and surrogate decision making.

Qualifications

• Master’s degree in Social Work.
• PA LSW and/or LCSW required (or obtain within specified timeframe).
• Act 34, Act 33, and Act 73 background clearances required.
• Experience in hospital or health care setting preferred.

Responsibilities

• Assess patients and coordinate care with caregivers, families, and the interdiscplinary team.
• Develop safe transition plans from inpatient/ED and connect with post-discharge resources.
• Educate patients and families on post-discharge care and access to community supports.
• Provide crisis intervention and support for end-of-life discussions and decision making.
• Engage in professional development and contribute to department goals.
