Qualifications

• Bachelor’s Degree in Nursing (RN) or equivalent.
• 3 years in a clinical nursing role.
• Current PA RN licensure or multi-state licensure via eNLC.
• Strong communication and collaboration with diverse teams.

Responsibilities

• Contributes to or completes comprehensive patient and family assessments.
• Identifies barriers and analyzes risk factors for safe transitions and care coordination.
• Coordinates clinical care with patients, families, providers, and interdisciplinary teams.
• Advocates for patients and families with providers, payers, and post-acute partners.
• Educates patients and families on care plans, documentation, and utilization.
• Pursues professional development and participates in committees.
