Job summary

John C. Lincoln Medical Center is seeking an experienced Licensed Social Worker to support the Pulmonary Hypertension Clinic in Phoenix. The role focuses on case management, discharge planning, and coordinating resources to ensure seamless transitions across care settings.

The ideal candidate has a Master of Social Work, 1 year of social work experience, and preference for case management experience. A sign-on bonus up to $5,000 is available.

Qualifications

• Master’s in Social Work required.
• 1 year as a Licensed Social Worker, or completion of MSW field placement.
• 2 years in case management preferred.
• 3 years as a Licensed Social Worker preferred.

Responsibilities

• Collaborate with patients and families for discharge planning and post-acute care.
• Identify readmission risks and arrange post-discharge support.
• Coordinate communication among hospitalists, specialists and PCP.
• Advocate for patients and ensure care complies with standards.
• Perform other related duties as assigned.
