HonorHealth is seeking a Licensed Social Worker to manage an assigned caseload and coordinate care across inpatient and outpatient settings. The role requires collaboration with patients, families, and a wide care team to ensure smooth transitions, appropriate resources, and cost-effective care.

Responsibilities include discharge planning, post-acute interventions, and ensuring timely communication with payers for authorization.

Competenze

• Masters in Social Work required.
• 1 year as a Licensed Social Worker, and/or successful completion of health related field placement in Master's level Social Work Program required.
• 2 years in case management preferred
• 3 years as a Licensed Social Worker preferred
• Other Experience in management of behavioral health patients preferred.

Mansioni

• Collaborates with patients/caregivers, healthcare providers, and external partners to ensure coordinated care and smooth transitions.
• Identifies and executes post-acute interventions including home visits and coordination of behavioral health services.
• Facilitates communication with payers for authorization and ensures adherence to standards of care.
