Job summary

Qualifications

• Master's degree in Social Work required.
• 2 years clinical social work experience in medical/clinical settings preferred.
• Strong interpersonal and coordination skills essential.

Responsibilities

• Perform comprehensive psychosocial assessments and develop discharge plans with RN Case Manager.
• Identify social/psychosocial issues affecting care and discharge planning.
• Develop, coordinate, implement and revise care plans to support transitions and cost reduction.
• Collaborate with physicians and multidisciplinary team to meet discharge goals.
• Provide supportive counseling to individuals, families, and groups.
• Complete assigned goals and provide crisis intervention as needed.
