The position emphasizes independent work with supervision from the Lead CM team, Supervisor, and Manager of Case Management, ensuring high-quality clinical services and effective transitions for patients and families.

Qualifications

• Bachelor's degree in Nursing or combination of Associate degree in Nursing with 3 years clinical experience.
• Master's degree in Nursing preferred.
• Minimum 1 year of broad clinical experience with care coordination, discharge planning and utilization review.
• Knowledge of third-party payer financial aspects and reimbursement preferred.

Responsibilities

• Provides comprehensive care coordination for assigned patients and assesses plan of care.
• Manages length of stay and discharge planning; develops and documents resource utilization and progress.
• Collaborates with interdisciplinary team to implement and monitor treatment plans.
• Communicates with patients, families and payers to resolve barriers to discharge and ensure transitions.
• Documents and reviews care plans, and escalates issues per department protocols.
