Qualifications

• Minimum Experience in hospital or health care setting.
• Master's degree in Social Work required for this role.
• PA LSW and/or LCSW credentials required (by 04/21/2025/12 months grace).
• Clearances: Act 34, Act 33, Act 73 background checks required.

Responsibilities

• Contributes to and completes initial and ongoing comprehensive assessment with patients and families.
• Implements recommended care plans coordinating with interdisciplinary team.
• Educates patients and families on post-discharge care and community resources.
• Provides crisis interventions for sensitive cases including abuse and mental health concerns.
• Supports professional growth through certification, mentoring, or committee work.
• Performs other duties as assigned.
