UBA at Montefiore Medical Center in Bronx, NY, seeks a dedicated bachelor’s or master’s level individual with social services experience to serve as a case planner for the Family Treatment & Rehabilitation program funded by NYC ACS.

This community-based, family-centered program collaborates with parents, children, and providers (including ACS) to support child safety and well-being. Responsibilities include engaging with families via home and school visits, case management, assessments, referrals, monitoring treatment engagement, and coordinating with community agencies and legal systems.

The ideal candidate will have experience with child welfare, mental health, or substance use issues, and working with families and legal systems. Excellent organizational and communication skills are required. Spanish-speaking ability is strongly preferred.
