Position Overview

University Behavioral Associates (UBA) at Montefiore Medical Center in Bronx, New York, is seeking a dedicated and organized bachelor’s- or master’s-level individual with experience in social services to serve as a case planner for the Family Treatment & Rehabilitation specialized preventive program funded by the NYC Administration for Children’s Services (ACS).

Responsibilities

1. Engage with families in the community through home, school visits, and other contacts.
2. Conduct ongoing in-home assessments and make referrals to clinical services.
3. Monitor attendance and engagement in treatment programs.
4. Collaborate with families, community agencies, and legal systems.
5. Ensure coordination among families, providers, and legal entities.
6. Participate in weekly supervision and document treatment efforts as per guidelines.

Qualifications

• Experience with child welfare, mental health, and/or substance use concerns.
• Experience working with families and legal systems.
• Excellent organizational and communication skills.
• Spanish-speaking skills are strongly preferred.
