Job Description: Case Planner at University Behavioral Associates (UBA)

University Behavioral Associates (UBA) at Montefiore Medical Center in Bronx, New York, is seeking a dedicated and organized individual with a bachelor's or master's degree and experience in social services to serve as a case planner for the Family Treatment & Rehabilitation specialized preventive program funded by the NYC Administration for Children’s Services (ACS).

This community-based, family-centered program collaborates closely with parents, children, and providers (including ACS) to support families in focusing on child safety and well-being. The case planner will primarily engage with families through home and school visits, as well as other community contacts.

Responsibilities

1. Work closely with families involved with ACS due to mental health or substance use concerns, including legal involvement.
2. Conduct ongoing in-home assessments of family members.
3. Make referrals to appropriate clinical services.
4. Monitor attendance and engagement at mental health or substance use disorder treatment programs.
5. Collaborate with families and community agencies (e.g., schools, probation, ACS).
6. Ensure coordination between families, providers, and the legal system.
7. Participate in weekly individual supervision.
8. Document treatment efforts in accordance with ACS and UBA/Montefiore guidelines.
9. Availability for evening and/or weekend work may be required.

Qualifications

• Experience with child welfare, mental health issues, and/or substance use concerns.
• Experience working with families and the legal system.
• Excellent organizational and communication skills.
• Spanish-speaking skills are strongly preferred.
