Position Overview

SOCIAL WORK ASSISTANT (CASE PLANNER)

University Behavioral Associates (UBA) at Montefiore Medical Center in Bronx, New York, is seeking a dedicated and organized individual with social services experience, holding either a bachelor's or master's degree, to serve as a case planner for the Family Treatment & Rehabilitation specialized preventive program funded by NYC ACS.

Responsibilities

• Engage with families through home and school visits, and community contacts.
• Manage cases involving families with mental health, substance use, or legal issues.
• Conduct ongoing in-home assessments and make referrals to clinical services.
• Monitor engagement in treatment programs and collaborate with community agencies.
• Coordinate between families, providers, and legal systems.
• Document efforts according to ACS and UBA/Montefiore guidelines.
• Possibility of evening and weekend hours.

Qualifications

• Experience with child welfare, mental health, or substance use issues.
• Experience working with families and legal systems.
• Excellent organizational and communication skills.
• Spanish proficiency strongly preferred.

Additional Details

• Salary: $32.18/hour
• Location: Bronx, NY
• Employment: Full-time, regular
• Shift: Day, 8:30 AM - 5 PM
