Genesis HealthCare's Pine Point Center in Scarborough, ME seeks a Social Services Specialist to identify patients' social, emotional, and mental health needs and coordinate services to enhance quality of life. Qualifications

• Bachelor's degree in social work or human services required.
• Must possess any state-required certifications/licensures to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well‑being.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others to provide support and information.
• Perform discharge planning activities and referrals to appropriate agencies and suppliers.
• Establish relationships and maintain referral flow with community-based agencies/services.
