Job summary

Genesis is seeking a Social Services Specialist at a nursing center to support patients and residents and their families by identifying social, emotional, and mental health needs and coordinating services to enhance overall well-being and quality of life.

You will work within an interdisciplinary team, contribute to care planning, conduct meetings with families, and facilitate discharge planning and connections to community resources.

Qualifications

• Bachelor's degree in social work or human services field required.
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in development of an interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others to provide support and information to improve quality of life.
• Perform discharge planning activities including education, documentation and referrals.
• Establish relationships with community-based agencies and services to benefit the patient/resident population.
