Job summary
Genesis is seeking a Social Services Specialist for our nursing center in Maine. You will work with patients, residents and their families to identify social, emotional, and mental health needs and arrange services to improve quality of life.
Collaborating with the interdisciplinary team, you will help develop care plans, hold meetings, coordinate discharge planning and connect with community agencies to support our patients and residents.
Qualifications
- Bachelor's degree in social work or human services field required.
- Must possess any certifications/licensures as required by State of employment to practice in long-term care.
Responsibilities
- Enrich lives as a nursing center Social Services Specialist where you will work with patients/residents and their families to identify needs and obtain services.
- Collaborate with the interdisciplinary team to promote patient/resident rights and well-being and educate staff on social services.
- Develop written interdisciplinary plan of care for social service needs.
- Hold meetings with patients/residents, families, and significant others to provide support and information.
- Perform discharge planning and make referrals to appropriate agencies and suppliers.
- Establish relationships and maintain referral flow with community-based agencies/services benefiting patients/residents.
