Genesis Nursing Center in Charles Town, WV, is hiring a Social Services Specialist to enhance residents’ well-being through coordinated social and psychosocial care.

You will work with patients, families, and the interdisciplinary team to identify needs, develop care plans, and arrange services, with meetings and discharge planning.

Qualifications include a Bachelor’s in social work or related field (preferred) and state licensure as required for long-term care.

Qualifications

• Bachelor's degree in social work or human services preferred.
• Must possess any certifications/licensures as required by State to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote patient/resident rights and well-being.
• Develop a written interdisciplinary plan of care for each patient/resident addressing social service needs.
• Hold meetings with patients/residents, families, and significant others to provide support and information to improve quality of life.
• Perform discharge planning activities such as education, documentation, and referrals to appropriate agencies.
• Establish relationships and maintain referral flow with community-based services that benefit our patient/resident population.
