Job summary

Genesis HealthCare, Inc. is seeking a Social Services Specialist at one of our nursing centers in Alabama to coordinate resident social services, support families, and advocate for patient well-being.

You will work with the interdisciplinary team to develop care plans, conduct meetings, manage discharge planning, and connect with community resources to enhance quality of life for residents.

Qualifications

• Bachelor's degree in social work or human services required.
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and referrals to appropriate agencies.
• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
