Job summary

BayCare Health System in Lutz, FL is seeking a Social Services Specialist II to join the team on a PRN basis. The role focuses on therapeutic interventions, coordination and discharge planning for patients across the hospital setting.

Ideal candidates hold a master's in social work (MSW preferred) with at least 1 year of hospital social work experience or a 6-month internship. Certification in case management is preferred. This position supports a patient-centered, multidisciplinary approach.

Qualifications

• Required Masters in Social Work, Sociology, or Related Field.
• Strongly Preferred Masters Degree in Social Work.
• Required 1 year Social Work in an acute care or hospital setting.
• Internship of 6 months in Social Work.

Responsibilities

• Provide therapeutic interventions through assessment, coordination, referral and interdisciplinary planning.
• Demonstrates knowledge of discharge and transition planning and linkage with community resources.
• Collaborate with patients, nurses, hospital care coordinators, physicians, caregivers and the community.
• Demonstrates skills in planning, organizing, and managing multiple functions and complex processes.

The Social Services Specialist responsibilities include:

• Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community

• The Social Service Specialist II provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning

• They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs

• Possess a clear understanding of discharge and transition planning and linkage with community resources

• Demonstrates skills in planning, organizing, and managing multiple functions and complex processes

Position details:

• Location: St. Joseph's Hospital North, Lutz, FL

• Status: PRN

• Schedule: 8:00 AM - 4:30 PM

• Weekend requirement: Occasional

• On Call: No

Certifications and Licensures:

• Preferred ACM (Case Management) or

• Preferred CCM (Case Manager)

Education:

• Required Masters in Social Work, Sociology, or Related Field

• Strongly Preferred Masters Degree in Social Work

Experience:

• Required 6 months Internship or

• Required 1 year Social Work in an acute care or hospital setting
