BayCare in New Port Richey, FL is seeking a Social Services Specialist II to provide therapeutic interventions and coordinate discharge planning within a collaborative care model.

This PRN role supports patients, families and care teams, with opportunities to connect individuals with community resources across shifts.

Formación

• Masters in Social Work or related field required.
• Strongly preferred MSW degree.
• Minimum 6 months internship or 1 year social work in acute care.

Responsabilidades

• Be part of a collaborative practice model with patients, nurses, physicians and care teams.
• Provide therapeutic interventions through assessment, coordination and referral.
• Coordinate discharge planning and linkage with community resources.
• Demonstrate planning and organization across multiple complex processes.

The Social Services Specialist responsibilities include:

• Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community
• The Social Service Specialist II provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning
• They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs
• Possess a clear understanding of discharge and transition planning and linkage with community resources
• Demonstrates skills in planning, organizing, and managing multiple functions and complex processes
• Location: North Bay Hospital - New Port Richey, FL
• Status: PRN, As Needed
• Schedule: Days Vary, 8:30 AM - 5PM
• On Call: No

Certifications and Licensures:

• Preferred ACM (Case Management) or
• Preferred CCM (Case Manager)

Education:

• Required Masters in Social Work, Sociology, or Related Field
• Strongly Preferred Masters Degree in Social Work

Experience:

• Required 6 months Internship or
• Required 1 year Social Work in an acute care or hospital setting
