Job summary

BayCare is seeking a Social Services Specialist II (PRN) at St. Joseph’s Hospital in Tampa, FL. The role supports discharge planning and care coordination within a collaborative, interdisciplinary team.

Schedule is days varying, 8:00 AM–4:30 PM, with occasional weekend coverage and no on-call duties. Minimum requirement includes a Master’s degree in Social Work or related field, with preferred CCM/ ACM certification. 6 months internship or 1 year hospital social work experience are expected.

Qualifications

• Required Master’s in Social Work, Sociology, or related field.
• Strongly preferred Master’s in Social Work.
• Experience: internship or hospital social work setting.

Responsibilities

• Member of a collaborative practice model with patients, nurses, care coordinators, physicians, and community members.
• Provide therapeutic interventions through assessment, coordination, referral and interdisciplinary planning.
• Understand discharge and transition planning and linkage with community resources.
• Demonstrate planning, organizing, and managing multiple functions and complex processes.

The Social Services Specialist responsibilities include:

• Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community
• The Social Service Specialist II provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning
• They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs
• Possess a clear understanding of discharge and transition planning and linkage with community resources
• Demonstrates skills in planning, organizing, and managing multiple functions and complex processes

Certifications and Licensures:

• Preferred ACM (Case Management) or
• Preferred CCM (Case Manager)

Education:

• Required Masters in Social Work, Sociology, or Related Field
• Strongly Preferred Masters Degree in Social Work

Experience:

• Required 6 months Internship or
• Required 1 year Social Work in an acute care or hospital setting
