Job summary

Qualifications

• MSW or related field master’s degree required.
• Minimum 6 months internship or 1 year hospital social work experience.
• ACM or CCM certifications preferred.
• Experience in acute care or hospital discharge planning is essential.

Responsibilities

• Be a member of a collaborative practice model with patients, nurses, care coordinators, physicians and others.
• Provide therapeutic interventions through assessment, coordination, referral and interdisciplinary planning.
• Understand discharge/transition planning and linkage with community resources.
• Demonstrate skills in planning, organizing and managing multiple functions and complex processes.

Social Services Specialist responsibilities include:

• Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community
• The Social Service Specialist II provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning
• They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs
• Possess a clear understanding of discharge and transition planning and linkage with community resources
• Demonstrates skills in planning, organizing, and managing multiple functions and complex processes

Position details:

• Location: St. Josephs Hospital - Tampa, FL
• Status: PRN, As Needed
• Schedule: Days will vary; 8:00 AM - 4:30 PM
• Weekend requirement: Occasional
• On Call: No

Certifications and Licensures:

• Preferred ACM (Case Management) or
• Preferred CCM (Case Manager)

Education:

• Required Masters in Social Work, Sociology, or Related Field
• Strongly Preferred Masters Degree in Social Work

Experience:

• Required 6 months Internship or
• Required 1 year Social Work in an acute care or hospital setting
