Job summary

Brickyard Healthcare seeks a Director of Social Services to lead resident social, emotional, and psychological support at our Newburgh facility. You will develop social histories, assess needs, and guide care planning to promote residents' well-being.

The role requires a Bachelor's in social work, current CSW license, and at least two years in a healthcare setting; you will collaborate with interdisciplinary teams to optimize discharge planning and resident outcomes.

Qualifications

• Bachelor's degree in social work required.
• Current Licensed Social Worker per state requirements.
• Minimum two years social work experience in healthcare preferred.

Responsibilities

• Orient residents to the long-term/sub-acute care environment and assist with facility adjustment.
• Develop social histories and care plans identifying problems, goals, and actions at admission.
• Document progress notes related to each resident's care plan within policy timeframes.
• Implement staff approaches addressing problems and periodically review effectiveness.
• Evaluate the psychosocial assessment and changes in resident needs.
• Revise the plan of care according to needs.
• Participate in interdisciplinary care plan meetings and assess discharge potential.
• Assist residents and families with discharge and placement planning.
