Job summary

Signature HealthCARE at Colonial Rehab & Wellness Center in Bardstown, KY, seeks a Social Services Director to identify and address residents’ social, emotional, and psychological needs, and guide discharge planning.

You will ensure compliant documentation, support resident care planning, and act as a resource for residents and families while upholding resident-centered values and confidentiality.

Qualifications

• Bachelor’s or Master’s Degree in Social Work or Psychology.
• 3+ Years Experience working in Social Services, directly with individuals, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Identify and provide for each resident’s social, emotional and psychological needs.
• Assist in the planning for each resident’s discharge.
• Participate in resident care planning and discharge planning.
• Serve as a general information source for residents and families.
• Ensure timely completion of documentation to comply with Facility, State, and Federal policies.
