Responsibilities

• Plan, organize, implement, evaluate, and direct social services per federal, state and local standards and facility policies.
• Meet with administration and medical staff to plan social services and ensure resident needs are met.
• Develop and maintain a good working rapport with intra-department and outside agencies to maintain social service programs.
• Assist in developing, administering, and coordinating department policies and procedures.
• Identify the medically related social and emotional needs of residents and participate in planning.
