Job summary

Signature HealthCARE is seeking a Social Services Director for our Bowling Green facility. You will identify and address residents' social, emotional, and psychological needs, and support the ongoing development of their potential during their stay and in discharge planning.

The role is part-time on weekends. Requires a BA/BS or MS in Social Work or Psychology, 3+ years of experience in social services in healthcare, and a high level of professionalism with strict confidentiality.

Qualifications

• Bachelor’s or Masters Degree in Social Work or Psychology.
• 3+ Years Experience working in Social Services, directly with individuals, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Assist residents and their families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
• Participate in resident care planning.
• Participate in the development of Discharge planning and prepare resident for the transition.
• Serve as a general source of information for residents and their families.
