Job summary

Brickyard Healthcare is seeking a Director of Social Services in Evansville, IN to oversee social, emotional, and psychological resident needs and support full-potential development during stay. You will lead social history development and care planning within an interdisciplinary team and assist with discharge planning.

Requirements include a CSWE-accredited Bachelor's in Social Work and current state licensure, with at least two years of healthcare social work experience preferred.

Qualifications

• Bachelor's degree in social work from a CSWE-accredited program.
• Current licensure as a Social Worker in the state of practice.
• Minimum two years of social work experience in healthcare preferred.

Responsibilities

• Orient residents to the long-term care environment and assist with facility adjustment.
• Develop social history, assessment, and care plans addressing identified problems and goals.
• Document progress notes related to each resident's care plan within policy timelines.
• Implement staff approaches addressing needs and periodically review effectiveness.
• Evaluate psychosocial assessments and staff approaches for improvements.
• Revise care plans as needed and participate in interdisciplinary Care Plan meetings.
• Assist residents and families with discharge and placement planning.
