Qualifications

• Minimum of one year of supervised social work experience in a healthcare setting.
• Experience working with the psychosocial needs of the elderly is preferred.
• Knowledge of nursing home regulations and reimbursement guidelines is a must.

Responsibilities

• Develop, plan, implement, and evaluate social service programs in accordance with state and federal regulations.
• Identify and address residents' social, emotional, and psychological needs.
• Coordinate services to support resident well-being and successful care outcomes.
• Collaborate with interdisciplinary team members to ensure holistic resident care.
• Maintain compliance with nursing home regulations, including MDS procedures and Medicare/Medicaid guidelines.
