Job summary

Atrium Centers Mulder Health Care in West Salem, Wisconsin, is seeking a Social Services Assistant to support residents’ social, emotional, and psychosocial needs within interdisciplinary teams.

You’ll assist with care planning, documentation, and regulatory compliance, while engaging families and residents with compassion and strong communication.

Associate degree preferred, experience in long-term care valued; 100% employee-owned ESOP culture and opportunities for growth are offered.

• Associate's Degree preferred but not required; experience may substitute.
• Experience in skilled nursing or long-term care is preferred.
• Strong written and verbal communication with residents and families.
• Ability to assist with documentation, assessments, and care planning under supervision.

Responsibilities

• Assist the Social Services Director in implementing social service programs per guidelines.
• Support identification and documentation of residents' social, emotional, and psychological needs.
• Provide service coordination and follow-up for residents and families.

• Maintain accurate documentation to support compliance and regulatory standards.

What You Bring

• Associate’s Degree in a Human Services, Social Work, Psychology, or related field preferred, but not required (experience may be considered in lieu of degree)

• Experience working in a skilled nursing or long-term care setting preferred

• Strong written and verbal communication skills, with the ability to interact professionally with residents, families, and interdisciplinary teams

• Ability to assist with documentation, assessments, care planning, and regulatory requirements under the direction of a licensed Social Services Director

• Compassionate, resident-focused approach with strong interpersonal skills

• Organized and detail-oriented, with the ability to manage multiple priorities

• Working knowledge of resident rights, discharge planning, and psychosocial support services preferred

• Positive, can-do attitude with excellent attendance
